Derek and Jo enter Jimmy's room, where Jimmy and his sister Carla greet them. Jimmy confesses that he tried to find another surgeon while Derek's hand was healing, but no one would go near his neuroma. Derek says they need to take scans to see if the neuroma has gotten worse. Outside, Derek wants Jo to switch with Shane, as Ross knows what Derek wants before he asks it and he won't have time to teach her on this case. As Jo walks away, Owen introduces Derek to Alana. She asks if pre-op overnights stays are typical, but Derek says she should ask Owen. He then walks away to go prep. She says she'll see him in the OR tomorrow. He turns around, and says she typically needs to be invited. \"If things were running typically, I wouldn't be here, now would I\" she asks. Derek continues walking, and Owen and Alana continue their tour.
Cristina, Alex, and Arizona are operating. As Arizona cuts into the patient, her left leg hurts. It's bleeding. Alex and Cristina barely react, and she calms herself down. When she continues to cut, her leg starts bleeding more. Alex and Cristina don't react as she tore open her pants, revealing two big cuts in her leg. As the patient starts crashing, Alex and Cristina watch how Arizona takes a scalpel and starts cutting off her leg. Arizona wakes up from the horrible dream, and she gasps.
Clark arrives at the convention and asks Lois the point of the whole thing. Lois explains that it is fantasy and it is meant to take people out of their ordinary lives and give them a chance to see the world in a new way. She asks him what his dream is, but Zatanna interrupts from behind them and asks Clark if they can speak alone. Clark and Zatanna head off, leaving a clearly jealous Lois behind.
Walden finds out about all the jingles that Charlie wrote and sings them out loud, causing Alan's grief to get the better of him. When he finds out that Walden donated his brother's piano, an object that has many memories of the latter tied to it, Alan drowns his sorrows with alcohol at a local bar. After watching Walden picking up another woman named Alicia (Amanda Schull), he returns home with them and steps out on the deck, looking up into the night sky as he talks to Charlie through the Heavens. When a labrador runs up to him, followed by its owner, a beautiful woman named Melanie (Taylor Cole), he suddenly starts to behave and talk like Charlie and even manages to seduce her. Berta remarks that \"it's walking like Zippy, but it sure ain't talking like Zippy\". Walden and Jake become worried about him after he starts to wear bowling shirts and calls himself Charlie Harper. He even walks in on Jake and Megan (Macey Cruthird) making out and ends up telling them to use protection. Walden then takes him to a mental hospital by pretending that they are going to Vegas. While lying in bed in the hospital, Alan, believing he is in the presidential suite of the Bellagio, orders two Asian prostitutes from room service, sits back with his cigar and declares that he is \"winning\", thus referencing Charlie Sheen's public statements after he was fired.
It is then revealed that Alan is still in the hospital and everything that happened was just a dream. When he actually returns home, he is confronted with Walden's new and modernized house decor. Much to his sadness, it no longer reflects Charlie's home, prompting him to admit that he no longer belongs here. But when Walden asks him if this means he is going to move out, Alan declines immediately. Gary Busey guest stars as Alan's clinic roommate.
Meanwhile, Elaine needs Puddy's coat back from Newman, so she reluctantly comes on to \"Newmie\" to get it. It's Newman's dream that his unrequited love wants him, but he rejects her advances and admits that he's in love with Silvio's wife. Now, it's up to Kramer to stop Silvio from evicting Newman despite the fact that he's sleeping with Silvio's wife, although admitting there is \"very little sleeping going on.\"
Newman contests a speeding ticket and uses Kramer as his witness. However, Kramer was recently kicked in the head by Joe Davola and now has a walking concussion. Newman tries to get out of the ticket by claiming he was racing home from Westchester, where he was playing Parchesi with an old blind man, to stop Kramer from committing suicide over the fact that \"he never became a banker.\"
